About this Form

Use this form if you are entitled to withdraw savings from your KiwiSaver account for retirement purposes and want to stop making further contributions.

When to use this form

If you have reached the age of 65 you have become eligible to withdraw your KiwiSaver savings.

You can stop your KiwiSaver deduction from their salary at any time after turning 65. This decision is optional.

What you will need

• Your name and IRD number
• Your employer’s name and address
