When to use this form

• If you are applying to Inland Revenue for parental leave payments for a child who you have permanent primary responsibility for.
• Only if you have no formal documentation such as a court order or letter from the Ministry of Social Development.

• evidence from your lead maternity carer of your due date or the baby’s date of birth
