About this Form

A form for businesses that officially cease trading to use to finalise their tax liabilities. In most cases your business can be ceased over the phone. This is faster than printing the form and sending it in.

This form can be completed on-screen by typing content directly into the PDF document. Once you have completed the form it is important that you print it out as you will not be able to save the information you enter into the form.

When to use this form

Use this form if you are in business and wish to deregister.

What you will need

Your business IRD number, the cessation date, business details relating to company, employer, fringe benefit tax and GST.

After you finish

Please continue to file all returns until we confirm that we have ceased your business.
